Simple Steps for a Smooth Experience

Preparing for Your First Appointment

After scheduling your appointment, you’ll receive a text message with a link to our New Patient Forms. Please complete these before your visit to save time.


To ensure your first visit goes as smoothly as possible, please bring the following:

  • Referral slip and X-rays (if applicable).
  • A list of medications you are currently taking.
  • Information about any known allergies or medical conditions.
  • Medical or dental insurance details for filing purposes.



If you’ve had X-rays taken in the last six months, ask your previous dentist to forward them to our office. If they can’t be sent in time, you may bring them with you. Patients under 18 must be accompanied by a parent or guardian.

Transparent & Honest Billing Practices

Understanding Payment & Insurance

We are committed to giving you specialized care at a reasonable fee, and we strive to estimate your treatment costs upfront so you can be fully prepared. Most insurance companies respond within 4-6 weeks, and you’ll receive a monthly statement.


Please note:

  • You’re responsible for any fees not covered by insurance.
  • Additional tests (e.g., lab work or biopsies) will be billed directly by the provider.


For your convenience we accept American Express, Visa, MasterCard, and Discover. As a courtesy to our patients who have dental insurance, we will complete and submit your claim to your insurance carrier for you. If needed, payment arrangements can be discussed before treatment begins.
